Preschool Forms

Below are all of the Preschool forms that are required to enroll your child in our school or that may be needed during the school year. The majority of these forms can be found in the registration packet that was given to you when you first enrolled, but in case you need one, you can click on the form below and print it out from your own computer.

Child Enrollment Form / Immunization Information

To complete enrollment, every child must have a current immunization record on file with the preschool. Parents or legal guardians are responsible for providing accurate and current immunization documentation before their child begins attending the preschool and for submitting updated records whenever additional immunizations are received, as required.
